TAVI and coronary interventions

Heberto Aquino Bruno, Giuseppe Tarantini and colleagues explore how coronary artery disease impacts transcatheter aortic valve implantation (TAVI). This State-of-the-Art reviews the evaluation and management of coronary artery disease, the indications for and timing of percutaneous coronary intervention before and after valve implantation, and strategies for managing coronary obstruction.

Early HVD after TAVI

Antonin Trimaille, Josep Rodés-Cabau and colleagues report on the incidence, predictors, and prognostic impact of early haemodynamic valve deterioration (HVD) in TAVI patients. They identify several independent predictors of early HVD as well as an association with a high rate of leaflet thrombosis at 1 year. The findings suggest that early postprocedural haemodynamic follow-up may have prognostic implications, as discussed by Ron Waksman and Asa Phichaphop</b> in an accompanying editorial.

Early outcomes of redo-TAVI with SAPIEN 3

The 30-day procedural and clinical outcomes of redo-TAVI with a SAPIEN 3 valve are examined by Giuseppe Tarantini, Radoslaw Parma and colleagues in the multicentre ReTAVI registry. Symptomatic patients with a failed prior transcatheter heart valve (THV) undergoing redo-TAVI with the balloon-expandable valve are evaluated according to Valve Academic Research Consortium 3 criteria at 1 month. A high procedural success rate plus low mortality and stroke rates, combined with favourable haemodynamic outcomes, indicate that redo-TAVI with the SAPIEN 3 THV platform is safe and effective.
