Pre-hospital crushed prasugrel tablets in patients with STEMI planned for primary PCI

COMPARE CRUSH
Objective
to assess the clinical efficacy of pre-hospital administration of crushed versus integral tablets of prasugrel in patients with STEMI scheduled to undergo PCI
Study
investigator-initiated multicentre randomised trial
Population
patients with STEMI receiving pre hospital crushed or integral 60mg prasugrel orally in addition to aspirin (500mg i.v) and heparin (5000 IU i.v). DAPT was recommended for 12 months.
Endpoints
proportion of patients with TIMI 3 flow was initial coronary angiography and the proportion of patients with ≥70% ST-segment resolution 1 hour after PCI. Safety endpoint: TIMI major bleeding and BARC bleeding ≥3 within 48 hours
Conclusion
pre-hospital administration of crushed prasugrel tablets does not improve TIMI-flow 3 or ST-resolution compared to integral prasugrel tablets
